Tailored support, bespoke accommodation

Phoenix House is an award-winning service that offers a large variety of accommodation and communal spaces. There are four spacious two-bedroom flats, each with a bedroom and ensuite, an additional bedroom and ensuite for waking night/sleep-in support, living room, dining area, and kitchen.

Two further transition flats offer a spacious bedroom and ensuite, living room, dining space, and kitchen. The transition flats are ideal for those looking to move towards more independent living. The site also has a three-person shared house. Everyone who lives at Phoenix House also has access to the communal kitchen, dining room and living space, offering a mix of personal and communal living which is ideal for socialising and taking part in activities. 

Our Residential Model

Our residential provision is far removed from traditional models of support— with a focus on community integration, proactive development of independence and Individual Care Pathways.

Our qualified team members provide each person with the care and support that they need, 24 hours a day, 7 days a week – providing a safe, positive and stimulating environment. Services are carefully designed to meet the needs of the individuals who live there —adapting provision to meet individual need, whilst ensuring staffing structures and training are built around each person.

Our recruitment process is focussed on values, ensuring that we only employ people who believe in kindness, involvement, quality of life and progress. We provide ongoing training, support and mentoring to ensure that our team have the skills required to deliver high quality care.

Tiered Support

This service is part of our tiered support model, providing a complete Transition Care Pathway from Residential to Supported Living services on the same site. For people with complex needs, or those who struggle with change, this care pathway creates a balance between familiarity and what is new and decreases anxieties around change. This is proven to facilitate smoother and more successful transitions.

Our Clinical Support Team

In addition to our highly trained support teams, we also have a specialist in-house Clinical Support Team. The Clinical Support Team is a team of specialists from different disciplines, who work closely together to deliver additional support and expertise to the people we support at Precious.

By having an in-house team, we bring together professionals and practitioners from across different sectors to deliver person-centred, holistic care which focuses on the needs of the individual. With no need to apply and wait for support from external teams, we can provide this specialist support quickly and efficiently.

PBS

At Precious, we are committed to improving the support we provide by embedding Positive Behaviour Support (PBS) throughout the organisation. At its very core, PBS is about treating people with respect, putting their needs at the centre and enabling them to achieve the quality and enjoyment of life that everyone aspires to.

PBS takes a person-centred approach to identify which areas of support will be most helpful to each individual. This includes working closely with the people we support to make sure that their choices and goals are heard, respected and actioned.

Referrals

We provide a rapid response to referrals and we will normally arrange a professional assessment within a few days of your call.

Our highly experienced team will carry out a full and detailed pre-admission assessment, free of charge and without obligation, prior to a placement being offered. This focuses on understanding the unique needs of each person and forms the basis of the their support plans if they move into the service.
